Internal audit report
Set out audit scope, methods and findings, rate each by risk, and track remediation so leadership and the audit committee see where controls fall short.
Prompt
Write an internal audit report on the procure-to-pay process at a mid-size US retail chain. Purpose: show leadership and the audit committee where controls fall short and drive remediation. Requirements: open with the overall rating and high-risk findings, state objectives, scope, period and sampling approach, define the risk rating scale, write each finding as criteria, condition, cause, effect and recommendation with a rating, include management responses with owners and target dates, and track the status of prior findings. Base every point on evidence, in an objective, measured tone.
